The following program documents require your acceptance and compliance in order to become, and maintain your status as, a LiftSquad Certified Installer. Please review and ensure that you are willing and able to meet these standards. When you are endorsed by LiftSquad, you can be proud of that accreditation and we devote ourselves to supporting your efforts in meeting our customers’ needs.
The online enrollment form will guide you towards becoming a full installation partner with LiftSquad. Over the next few pages, you’ll acknowledge our Installer Standards and the Installer Fee Schedule.
Before you begin, please make sure you have electronic versions of the following documents ready for upload:
- A completed W9 (available for download here)
- A Certificate of Insurance* prepared from your agent (see example)
- A copy of your Elevator License (if applicable).

* LiftSquad/Harmar requires a current certificate of insurance showing current liability insurance, as well as Automotive Coverage and Workers Compensation with Harmar listed as a named insured party. The limits of liability should be a minimum of $1,000,000. (Gen Aggregate $2,000,000, ). The company Harmar submits payment to must be listed in the description area.
